FCC form help

Does anyone know how to fill out the U.S. customs form for an HDX900 with nano flash being shipped to me from overseas?

It is called the "Statement Regarding the Importation of Radio Frequency Devices Capable of Causing Harmful Interference"

If anyone knows hot wo fill this out, please PM me, thanks!

actually I was thinking the opposite. If it is what I think it is it is more or less a routine form and as neither the 900 or nano flash have the capability to emit radio waves, or at least certainly not in a manner that could cause interference to anything else. I believe you are just stating that.
